For Patients

At Clermont, Obstetrics, Gynecology & Infertility, a division of Women's Care Florida, we welcome new patients and are pleased that you have chosen our practice for your health care needs. We know you have a lot of choices when it comes to finding an OB/GYN that’s right for you, which is why we’re so glad you stopped by our site to give us the opportunity to introduce you to our practice and our physicians. Please download and complete the Confidential Patient Registration form below and bring them with you when you come for your appointment.

You will also need to provide your insurance card so we can copy it and collect any applicable co-pay. If you do not have proof of insurance you will be required to make payment at the time of your visit. If you do not have insurance coverage you will need to contact our billing department at (352) 241-6460 prior to your visit.

Appointments

Appointments may be scheduled during our office hours Monday through Friday from 9 AM to 5 PM. We prioritize requests based upon the degree of urgency and make every effort to return your call as quickly as possible. Our physicians, nurses and office personnel, in an effort to better serve you, are available by telephone and we make every effort to return your call as soon as possible.

Appointments help us to provide you with the time and attention we know you deserve. We ask that you provide us with a 24-hour notice if you are unable to keep your appointment time and to reschedule your appointment for a time that is convenient to you. We ask that patients arriving late for their appointment be tolerant, allowing our staff to arrange to work you in as soon as possible. A parent or legal guardian must accompany patients who are minors at the time any services are rendered.
